CareHome offers residential care with 24-hour supervision, individualized care planning, assistance with activities of daily living, medication administration, life skills coaching, and opportunities for social and community engagement. We serve a diverse population of adult and elderly men and women, and tailor our care plans to meet the unique needs of each individual we support.

Our home is staffed by a highly trained, compassionate team of full-time professionals, including an LPN, a social worker, and an RN who is on staff 24/7, all with extensive backgrounds in residential group home care and the medical profession. Our team receives ongoing education in medical care, human rights, behavior support, emergency preparedness, CPR, HIPAA, incident reporting, mandated reporting, and person-centered planning, ensuring compliance with Virginia HCBS regulations and best practices.

We are fully licensed by the Virginia Department of Behavioral Health and Developmental Services (DBHDS) and maintain all required certifications, liability insurance, and compliance documentation.

Our Sterling home features three (3) beautifully furnished beds, offering a warm, inviting, and fully accessible setting. The residence includes wheelchair ramps and ADA-compliant access throughout, ensuring safety and independence for all residents. Each resident enjoys a private or semi-private room, thoughtfully decorated and equipped for comfort and a sense of personal space. The shared living areas — kitchen, dining room, and living room — are clean, welcoming, and conducive to fostering a sense of community and belonging. In addition, we maintain an ADA-compliant transportation vehicle to support residents’ medical appointments and community activities. The home is kept to the highest standards of cleanliness, safety, and aesthetics, with regular housekeeping and maintenance.

At our home, we understand that each resident deserves attentive, compassionate, and professional medical support in the comfort of a home-like setting. Our Skilled Nursing Care at Home services are designed to provide residents with the clinical expertise they need while preserving their dignity and quality of life.

Our licensed nurses deliver personalized medical care tailored to each resident’s unique health needs. Services may include medication management, wound care, vital sign monitoring, chronic disease management, post-surgical support, and coordination with physicians and specialists. We prioritize continuity of care, safety, and proactive health monitoring to help prevent hospitalizations and promote recovery and wellness.
